We can provide you, your family and/or carer with information on organisations and services that could help. This may include shops that do home delivery, meal delivery services, gardening services, volunteer support networks and care lines.
We may be able to arrange short-term support to help you settle back into your home or help you with difficult issues, such as housing or financial problems.
We may also be able to help you contact private care agencies who can support you in your own home.
If you need more intensive support, we will ensure you are referred to your local social services department. They will work with you to assess your needs and enable you to have as much independence as possible after leaving hospital. They will ensure you have choices about how your needs are met.
